A 60-year-old male cleaner presented with a 5-month history of symmetrical, well-demarcated, erythematous plaques on the inner thighs [Figure 1], accompanied by pruritus. Occupational exposure involved frequent kneeling and use of a chloroxylenol-containing cleanser (0.1%), which soaked through work pants. Patch testing with the cleanser “as is” elicited a 2+ reaction (ICDRG-International Contact Dermatitis Research Group criteria), while controls were negative. The cleanser’s pH (6.8) and absence of irritants excluded irritant contact dermatitis. Complete lesion resolution followed discontinuation of allergen exposure and treatment with topical fluticasone propionate 0.05% ointment (applied twice daily to affected areas for 2 weeks) combined with oral cetirizine 10 mg once daily. Symptoms resolved after discontinuation. This case highlights clothing-facilitated allergic contact dermatitis in non-occluded areas, emphasising mechanical friction and allergen retention in atypical occupational dermatitis.
