The importance of differentiating erosive pustulosis of the scalp from other diseasesAn otherwise healthy 92-year-old man presented with asymptomatic lesions on the scalp of six months’ evolution. On examination, extensive erosions and crusts were observed on the scalp [Figure 1]. Bacterial, fungal and acid-fast cultures were negative. A punch biopsy ruled out malignancies. He was diagnosed with erosive pustulosis of the scalp, and treated with astringent dressings and topical corticosteroids with complete response. Erosive pustulosis of the scalp is an infrequent chronic inflammatory dermatosis of uncertain aetiology, although it has occasionally been related to triggers such as local trauma. It is important not to confuse erosive pustulosis of the scalp with malignancies, to avoid performing highly invasive procedures.
