Giant cutaneous metastasis of gastric signet ring cell carcinoma

A 68-year-old man with a history of gastric adenocarcinoma presented with a rapidly enlarging cutaneous mass over the past month. Physical examination revealed a giant violaceous tumour with well-defined borders on the neck and back [Figure 1]. Histopathological examination revealed diffuse infiltration of tumour cells with characteristic signet-ring morphology in the dermis [Figure 2]. Tumour cells were positive for CK, CK5/6, CK18, Claudin-18.2, and CDX2 [Figure S1]. Immunohistochemistry confirmed cutaneous metastasis of gastric signet-ring cell carcinoma. Post-chemotherapy, the tumour decreased in size. Giant cutaneous metastasis of the gastric signet-ring cell carcinoma, an exceedingly rare manifestation, is associated with advanced-stage cancer and an extremely poor prognosis. Clinicians should maintain a high degree of suspicion for atypical skin lesions that occur in patients with a history of malignant tumours.
