Pachydermoperiostosis

A 40-year-old male presented with thickening and furrowing of skin over face and scalp with thickening of eyelids, cheeks and chin, deep nasolabial folds and thickening of skin over hands and feet for 25 years [Figure - 1]a. Seborrhea and open sebaceous pores were seen. Clubbing with terminal widening of fingers [Figure - 1]b and [Figure - 1]c and toes and palmoplantar hyperhidrosis were noted. There was widening of forearms and lower legs. A diagnosis of pachydermoperiostosis was made. X-rays of hands and feet revealed soft tissue swelling and irregular periosteal thickening of the distal end of long bones, metacarpals, metatarsals and phalanges [Figure - 1]d.
