Trichobacteriosis of scrotum

A 45-year-old obese man presented with yellowish discolouration of scrotal hair for 1 year associated with malodour, pruritus, and excessive sweating. Examination revealed firmly adherent yellowish concretions along scrotal hair shafts with foul odour [Figure 1a]. Wood’s lamp examination showed distinct green fluorescence of affected hairs [Figure 1b]. Dermoscopy (DL4, 10×, polarised) showed yellow nodular concretions on shafts [Figure 1c], while light microscopy revealed irregular brownish concretions coating hair [Figure 1d]. A diagnosis of Trichobacteriosis (formerly called as Trichomycosis pubis) was made which is is caused by Corynebacterium species. The patient improved with 1% topical clindamycin for 2 weeks and hygiene measures.
