A 2-year-old boy presented with congenital multiple whitish cysts along a cord-like swelling running in the midline extending from the anterior margin of the anus to the lower pole of the scrotum [Figure - 1]. Biopsy showed a dermal cyst lined by pseudostratified columnar epithelium.

Median raphe cysts are hypothesized to originate from a developmental defect of the male urethra occurring as single or multiple cysts in the midline anywhere from the anus to the urethral meatus. The canaliform variety is very uncommon but clinically distinct. Asymptomatic lesions are left untreated; symptomatic lesions may be surgically excised to prevent complications.
